Enrollment requirements
The following requirements must be submitted to the Registrar's Office before admission:
- Report Card/Form 137
- Certification of Moral Conduct
- Photocopy of Birth Certificate
- Four pcs. 2x2 Colored Photo (with white background)
Enrollment procedures
- 01Go to the Admission Office and fill out the Enrollment Application Form and present all the requirements for enrollment
- 02Proceed to the Foreign Students Office and submit the original and photocopy of SSP, ACR, Passport and or VISA
Foreign pupils only
- 03Proceed to St. Therese Building for enrollment evaluation
- 04Proceed to Window 7 (Accounting Office) for payment
- 05Proceed to Window 4 (Registrar's Office) for the computerized Official Registration Form
- 06Proceed to ID Section
