• Certification of Moral Conduct
• Photocopy of Birth Certificate
• Four pcs. 2x2 Colored Photo (with white background)

Enrollment Procedures

   Go to the Admission Office

   Fill out the Enrollment Application Form and present all the requirements for enrollment.

   Note: Foreign students only

   Proceed to the Foreign Students Office and submit the original and photocopy of SSP, ACR, Passport and or VISA.

   Proceed to St. Therese Building for enrollment evaluation

   Proceed to Window 7 for payment

   Accounting Office

   Proceed to Window 4 for the computerized Official Registration Form

   Registrar's Office
