Minimum requirements
Submit the following to apply:

- 2. Personal Data Sheet — CSC Form No. 212 Revised 2025 which can be downloaded at www.csc.gov.ph, fully accomplished and notarized, with recent passport-size picture.
- 3. Photocopy of certificate of eligibility / rating / license
- 4. Photocopy of Transcript of Records / Diploma
- 5. Certificate of Employment / Service Record
- 6. Certificate of relevant training/s attended
- 7. Certificate/s of Awards / Recognitions / Appreciations (if any)
- 8. Performance rating in the last rating period
Deadline and reminders
"APPLICATIONS WITH INCOMPLETE DOCUMENTS SHALL NOT BE ENTERTAINED."
The deadline for submission will be on March 25, 2026.
