For food handlers: Chest X-ray result (original and photocopy - valid for 6 months); Fecalysis / stool examination result (original and photocopy - valid for 1 month); 1x1 ID picture; Valid ID (photocopy). For non-food workers: Chest X-ray result; 1x1 ID picture; Valid ID (photocopy).

Health certificate fee: P50.00. For meat handlers: Additional P100.00.

Attendance in the seminar is mandatory before the health ID / certificate can be issued. Estimated processing time is 30-45 minutes, depending on the volume of applicants.

Mandatory ang pagdalo sa seminar bago maibigay ang health ID / certificate. Ang tinatayang oras ng pagproseso ay 30-45 minuto, depende sa dami ng mga aplikante.
